Physical and Dental Exams

Physical Exams

New York Education Law requires a health examination (physical exam) for school attendance for all new entrants and students in grades Pre-K, K, 1,3,5,7,9 and 11, every year for students participating in athletics, as needed for working papers and when required by the Committee on Special Education (CSE).

  • Physicals must be completed on the NYS School Health Examination form
  • Health appraisal forms, from a health care provider’s office, are accepted if they contain all the required information and are in the same format of the required NYS form.
  • Documents are required by NYS within 30 days of starting school.
  • Physicals must be signed or stamped by the provider and provide the date of the physical exam.

Dental Exams

New York Education Law requests a Dental Certificate for students in grades Pre-K, K, 1, 3, 5, 7, 9 and 11 and for all new entrants to the district.

Screenings

The West Irondequoit Central School District must comply with all the New York State laws and regulation related to school health.

During this school year, the following screenings are required or completed at school:

  • Vision Screening - Distance and near acuity for all newly entering students and students in Pre-K, K, 1, 3, 5, 7, and 11. Color perception screening for all newly entering students.
  • Hearing – Hearing screening for all newly entering students and students in Pre-K, K 1,3, 5,7 and 11.
  • Scoliosis – Scoliosis (spinal curvature) screening for all girls in grades 5 and 7 and boys in grade 9.                                                                                                                                                                                             

A letter will be sent home if there are any findings in the screening completed at the school that would cause concern or need medical follow-up.  Please call the school’s Health Office if you have any questions or concerns.

 Release of Health Information Authorization

Communication between private and school health staff is important for safe and effective care. Your healthcare provider may not share health information with the school health staff without your signed permission.  You may submit the district Release of Health Information Authorization form to the school health office to assist in future communication with your healthcare provider.
