TDaP Vaccine

Dear Parents/Guardians: New York State Public Health Law Section S7421B now requires the Tdap vaccine for students entering sixth grade on or after September 1, 2007. Specifically, students born on or after January 1, 1994 who are 11 years of age or older, must receive an immunization containing tetanus toxoids, diphtheria and acellular pertussis (Tdap). Children entering sixth grade who are 10 years of age must be immunized when they turn 11. New York State Public Health intent is to protect a child now and in the future. Tdap vaccine can protect adolescents and adults against three serious diseases. Your Health Care Provider will be able to assist you in obtaining the vaccine. Acceptable proof for this requirement includes the following: Written documentation of Tdap from a health care provider. A medical exemption consisting of a written statement from a health care provider, stating there is a valid medical contraindication to Tdap. The Monroe County Health Department walk-in-clinic (274-6051) offers free immunizations on Wednesdays between the hours of 12:00 p.m. and 7:00 p.m. No appointment is required but it is necessary for you to take your child’s immunization record to the clinic or ask for a written statement indicating the vaccine has been administered. Written verification from your Health Care Provider will be needed by September 1, 2007 for your child to attend school. Documentation is to be sent directly to the school nurse. The school can receive the information through a fax if permission has been given by the parent to the Health Care Provider. Please contact the school nurse if you have any questions.
