2009 Winter Sport Athletes Eligibility Forms / Physicals Available Interscholastic/Athletic
Eligibility Permission Forms and Authorization for Release of Medical
Information Forms will be available beginning October 12 in the Main Office
at IHS and in the Athletic Office at IHS. Forms for Dake sports will be
available in the Dake Main Office beginning October 30, 2009. Per NYS
Athletic Guidelines, forms cannot be dated before 30 days prior to the start
of a sport. Eligibility forms are required every sports season. Forms must be
returned to the Health Office 10 days before the sport begins.
*Students
who do not turn in forms by this date may not be cleared to play by the first
day of practice. As per NYS regulations, all physical forms must be reviewed
by the school physician. Do not delay in turning in your paperwork. All medical
excuses must also be cleared before athletes are eligible to play. Athletes must have a
current physical (dated within one calendar year of November 1, 2009 on file
in the school Health Office. A Parent Permission for a
Pre-Participation Sports Physical form is required. Sports physicals will
