SPORTS NYSED requires an annual physical examination for all new students entering 7th grade. Sports eligibility forms must be filled out for each sport season. The eligibility forms can be found in the main office over summer recess. Completed forms must be returned to Dake Main Office. Because this information will give us current health concerns, we ask that you submit the forms within one month of the start of the sport and no later than one week before the start. Current physical exams must be on file in the Health Office. These must be dated within twelve months of the start of the sport.
