Note about templates: 

Click on the template you wish to use.  Select the Save option.  Save the template to a convenient place on your network drive.  When you open it, it will be a near-exact copy of the new SPPT goals document, ready for you to complete.  Remember, this is a template.  When you have completed the document for yourself, you should name the file appropriately and save it.  The template remains unchanged and ready to reuse.
